Parent Letter – SLB/MenACWY

Dear Parent or Guardian

The Rotherham, Doncaster and South Humber NHS Foundation Trust (RDaSH) School Age Immunisation Team will be visiting your child’s school soon to complete Tetanus, Diphtheria and Polio boosters (3 in 1 teenage booster) and Meningitis ACWY immunisations to all students in Year 9 as part of the routine childhood immunisation programme . Please check with the school for the date.

Once you have read through the information, please return to the electronic consent form and complete all the required fields. Please make sure that you submit the form no later than one week before the vaccinations are due to be given to ensure your child receives their vaccine.

If you decide you do not want to vaccinate your child please complete the electronic consent form giving the reason. If you are unable to complete the form online, or have any questions you wish to discuss with a nurse please contact the Vaccination Team via our Single Point of Contact (SPOC) Tel: 03000 218997.

Failure to return the consent form will not be taken to mean refusal.

We are also able to offer MMR vaccine (Measles, Mumps & Rubella), which your child should have completed prior to starting in school. If you believe your child did not receive one or both doses of MMR and this is something you wish them to complete, please enter when asked this information on the electronic consent form, or if you are completing a paper copy contact us via the telephone number above and we can arrange for this vaccination to be given in school at the same time as your child’s other vaccines.
